There are four forms that must be filled out and submitted to a troop leader at the beginning of each Girl Scout year, Oct. 01.  They are the Girl Adult Health History Form, the Yearly Permission Form for Troop Meetings and Trips, the Code of Conduct for Troop Members and the combined Fall Product/Cookie Sale Permission Slip.
